How to Submit a Request:

Requests must be filed using the Department’s Public Records Request Form.

You can make a request for a police record using one of the options below:

  1. Phone:  Call the Records Division at 608-364-2984 Ext. 10
  2. Download the request form and fax or email the form;
    1. Fax:  Fax the Records Division at 608-313-0450
    2. Email:  Email the Records Division
  3. Complete the Online Request Form

Please Note: Record requests can take up to 10 days to fill and be prepared to pay a minimum of $.25 per page for your request.
